Information
Every player is required to have a physical
Physicals can NOT be dated 4 months prior to start of practice. They must be signed and stamped by the Physicians office. Physicals done by a Chiropractor or Registered Nurse are not accepted.
The weight on the physical must not be over 5% of the weight requirement for the player's division. See weight and age requirements on the divisions page.
Every player is required to have an original Birth Certificate
Birth Certificates are required to play. The Original copy is due by the first week of practice to your Athletic Director and will be returned after Paper Certification on TBA. No photo copies are accepted.
